Our support begins with a free call with our Intake Coordinator to gather some general information about your situation, answer any questions you may have about our services and will schedule an intake session with our Care Support Coordinator.

Our Care Facilitator will connect with you to explore what led you to our services and to learn better how we can help. Together, you'll identify the support that can best assist you and your family at this moment.

From there we will create a Personal profile and design a personalized Support Plan that will include educational tools, community resources, and professional options specific to your situation, budget, and current circumstances.

If you would like support in a number of areas or for a number of family members, you can meet with one of our Peer Mentors to create a Full Support Profile. Our Peer Mentors have had lived experience with your particular challenge and will offer compassion and understanding without judgement. They will meet with you to gain more insight into your experiences, needs, and hopes for the future and will help you explore what support might be helpful to you in all your areas of need.

You will also have access to the workshops we offer at a discounted fee.

“Life’s messy” and your family’s situation, ages, needs, finances, ideas and expectations change as time goes by. With that, the support you need will also change.

With our ongoing monthly membership, our hope is that we will become a safe place to reach out whenever you need, where we already know you and your family and can give you ongoing support, guidance and resources.

In addition to providing you with ongoing support, it includes a monthly check-in session via phone, zoom or in person to assess your needs and explore additional resources and services. You'll also enjoy full access to our online educational tools, videos, workshops, and webinars to support your journey.
